SMCC Rental Event Form
A refundable security deposit is required to book your event on the SMCC church calendar. Room rental prices vary according to campus. The security deposit is refundable if the facility and equipment is left in a clean and orderly manner. Once this form is submitted and payment is received, the reservation will be changed from a tentative to a confirmed status. Until all paperwork and payment(s) are received, the date is subject to booking by another event.

The rooms reserved at SMCC will be available as per the time agreed upon after on the Event Rental Form has been submitted. All room preparation, set up and clean up need to be done within that time period.

SMCC will work to accommodate the number of tables and chairs requested, but cannot guarantee all requests can be fulfilled due to other ministry needs.

Will there be food at this event?
The event contact person is responsible for the set-up, take-down and clean-up of their event. All rooms used are to be returned to their original state and "Sunday Ready". This includes: making sure all surfaces are clean, cleaning and putting away all coffee supplies, putting the chairs and tables back to their original position and taking out the trash/relining the trash cans. *
SMCC welcomes children to attend your event, but adult supervision is required at all times. Supervision is to be provided by the rental party. There will be no SMCC staff supervision. Parents must remain on the premises at all times.
Alcohol will be served in accordance with Utah state liquor laws and regulations. Wine, champagne, and beer are the only alcoholic beverages that may be served at SMCC and need to be offered at no cost to participants. If a cash bar is desired then a licensed caterer with blanket liability insurance must be used. If alcohol is brought on the premises without prior arrangements and is not removed immediately, you will forfeit your security deposit. SMCC reserves the right to end alcohol consumption and/or the event if problems arise.
Only electric candles may be used. They are to be in a container (glass or otherwise). Real flower petals may not be used. If silk or other artificial petals are used, they need to be removed as part of the cleanup. Nothing may be hung from the walls (no tape or pins/nails). All stage changes must be supervised and approved by an SMCC staff member. Use of rice, glitter or confetti inside or outside the building is prohibited. The SMCC facility must be left as it was found - clean and in order.
SMCC does not have a catering service so all food and beverage service will be the responsibility of those renting the facility. SMCC has no storage available, so nothing can be left overnight. Please plan accordingly.
Leave rooms used in the condition they were found. Trash is to be picked up and thrown in the dumpster. The event must be completely finished, guests departed, and clean up completed within the time frame of the rental agreement. Clean up must be completed immediately following the event; nothing can be stored in or on SMCC’s premises. If extra time is required or the premises are left in an unacceptable state, there will be an additional charge taken from the original deposit.
The undersigned agree(s) that South Mountain Community Church (SMCC) is not responsible for loss or damage to persons or property belonging to the undersigned or any of the undersigned's employees or guests using SMCC’s facilities and, thereby, releases SMCC from any claims for loss or damage to persons or property that arise out of or are connected with the undersigned's use of SMCC.
The undersigned agree(s) to defend, hold harmless and indemnify SMCC, employees and agents against all claims for loss, damage or injury sustained by persons or property arising out of or connected with the undersigned's use or occupancy of SMCC.
The undersigned is completely responsible for the facilities and property and assumes liability for any and all damages to SMCC or property, including personal property, that may occur as a result of the use of SMCC pursuant to this agreement.
I have read the online Rental Terms and Conditions and the Rental Rental Form above and agree to abide by the stipulations. I agree to obey all applicable federal, state and local laws and SMCC’s policies.
Once the form is submitted, you will be contacted about available room reservations, dates, costs and any other questions you might have. After that contact, all applicable fees are due. Once payment is received, the date of the event will be scheduled in the church calendar. Until that time, the date will only be held on a tentative basis and subject to booking by another event.
